This will allow sufficient time for the registration process. You may also bring the forms with you to at the time of your first appointment along with your photo ID and Insurance card(s).
Please arrive 15 minutes earlier than your scheduled appointment time so we can prepare your chart prior to your appointment. If you can’t keep your appointment, kindly give us 24 hours advance notice if it’s possible.
